I tend to find a picture I like and then just adjust the size numerous times, moving the image around until I get a crop I like. I might end up with 3 or 4 different icons from one image based on what I've resized it to or where I've positioned it.
The hard part is if I want something specific and it just doesn't work with a 100x100 space, then it's a case of going with another image or just fiddling until it's close enough to what I want that I can fudge the background.
I don't class myself as a good icon maker - I'm still learning techniques and my imagination for creating art is really limited, but I like giving it a go :) I just wish I could master text in icons!
